A Note About Membership - We accept membership applications on a continuing basis. The school year runs from July 1st to June 30th, so memberships taken after January 1st will be considered 1/2 year with expiration on June 30, 2011. Fees are $32.00 for a full-year and $28.00 for a half-year. These fees include your NY State LEAH dues which we are required to collect if Charity will be your primary LEAH group. You may download the application and information package here, or call Matt or Ellen Gerwitz at 594-4641 and we can mail it to you. (Remember: the download is free, mailing will cost Charity LEAH. Let's try to keep our expenses down.)

For Renewing Members: In order to make sure our records are up to date, we are no longer using short forms. Furthermore, we are updating our old long form to more closely reflect the information we must submit to the state organization. That form will be available for download beginning June 16th.
 
NOTE: When renewing your HSLDA membership or signing up for a new membership, you can receive a 15% discount as a LEAH member. When filling out the application use "LEAH" as the discount group name, and 292155 as the discount group number.

Finally, we ask you to please, please, please look up your ZIP+4 code to include on the application. You may find your ZIP+4, if you don't already know it, on the USPS website. It takes us approx 2 mins. to look up the info for you, so if we do that for just 30 people, we have spent 1 hour doing what you could do in two minutes. Please help us. Thanks.
